[RefChat] CYSAN send-off forms... Who is responsible for the form?

Ultimately CYSAN. It's their form. Until someone in a position
of authority points out the problem with the form to a person
within CYSAN who can correct the form, it will remain as it is.
It's been about 4 years since the last revision:

I would suggest first contacting Tom Dunlap, the D2YRA, and
bring the matter to his attention.

I might also suggest contacting Kathi Killion, as she is the
recipient of 24-hour send off reports for any state level youth
competition within D2.

In the immediate term, to fill out the form for coaches, use "C"
or "AC" for Coach or Assistant Coach to bypass the 3-character
limit for the jersey number, and use the Explanation field for details.
Even though coaches are not sent off, select one of the
following possible offenses which most closely matches why you
are expelling the coach:
Violent Conduct
Spit At Opponent Or Any Other Person
Offensive, Insulting or Abusive Language
Finally, Tom Dunlap and others are considering changes to the
send off report form, including tracking disciplinary actions
against participants, so please do send your comments and
suggestions to him.
