[RefChat] CYSAN send-off forms... Who is responsible for the form?

Who is actually responsible for editing/maintaining the CYSAN Send Off
Report form?  The web-based fill-in-the-blank form (and the PDF one, for
that matter) force the reporting referee to enter a jersey number and to
specify one of the seven send-off offenses, even if the person being sent
off is a coach.  As we all know, the only reason for which team officials
can be expelled is if they fail to conduct themselves in a responsible
manner.
