Updated CYSAN Referee's Send Off Report

Hello,

I have updated the CYSAN Referee's Send Off Report, both for the web-based and PDF versions.  Please use this form for reporting all CYSAN send offs effective immediately.

The form now includes fields for the official's email addresses, so it is a good idea to make a note of all of your crew's email addresses.

Additionally, fields have been rearranged and penalties are noted on the form such that disciplinary committees have an easy reference for determining penalties.
